Screen freezes and some rectagles cover all the screen, sort of greenish and violet

I use mac with both windows and mac os, the problem seems to persist only in mac os x while booting in windows seems to work just fine.
Screen freezes and some rectagles cover all the screen, sort of greenish and violet, after i restart they are gone, but then they appear again. Unless I login to windows where the screen is perfectly normal.

please help me fix the issue.
I dont think its a hardware issue, since it only shows after the system is started up, the grey screen is good also it works flawless in windows.
PLEASE HELP ASAP.

Year and model of your Mac. This is important especially if your Mac is a MacBook Pro with dual graphic processors.

Which version of OS X you're running.

Looks like scrambled video which indicates a serious problem with your graphics card (been there, done that, loads of repairs, new machine x2). There is a free repair programme but it depends what Mac you have. https://www.apple.com/uk/support/macbookpro-videoissues/

Type Model: Macbook pro mid 2012 13inch
Mac os X Sierra, also bootcamp installed with windows 10
SSD 240GB
No attached devices.
The issue happens only when logged in mac.
