I've attempted to burn several CD's from Itunes and I get a CD failed message just prior to the "writing phase" being completed. Is it time to replace the burner or is there some way to evaluate if the problem is related to hardware that is failing?

Without providing us with any info about your system - I'll venture a guess here that the burner hardware may be dirty or defective. Buy a can of compressed air and blow a few blasts in the drive and then try again to burn a disk.

Here's a summary of my equipment: 2005 Macbook Pro 2,2; Intel Core 2 Duo; 2.16 GHz processor speed; Disk Driver: Mat****aDVD-R UJ-857D.

I tried blowing out the DVD/CD with canned air as suggested and re-tried burning a disk. It acted like it was working until it got to the final phase of "writing" and then seemed like it got stuck in a loop with the drive making that humming noise, stopping, humming noise, stopping, etc. I finally tried ejecting the disk which it finally did after numerous attempts.

Well, your burner drive is likely dead - an internal replacement would be expensive, so your best option might be to buy an external CD/DVD unit - I use the Apple SuperDrive w/ my MBPro (cost me about $70), but less expensive choices are available - check Amazon, Other World Computing, or any of the other online electronic/computer sites. With the age of your computer, I'm not sure if there are any USB/software issues? Good luck! Dave :)
