In my personal hotspot I see a person's name, then the name iPhone, and another iPhone (3)!!!

Have I been hacked, or is someone using my wifi?

A hotspot is a hotspot, everyone can have one. If you want to connect to someone else's hotspot, you can, unless they have it secured with a password.

Basically, if you have no WiFi in a given location your device will "see" other devices that have an internet connection as available connections for your device, but, if you try to connect to one the owner need to approve the connection otherwise nothing happens.
