The new macbook problems (help!)

I got a new macbook a few days back, but my friend borrowed it since his was gone for repairs. After I got it back I refreshed it (complete reinstall).

Now the problem is, I see some apps in spotlight which aren't there, these I think were there when he used the mac.

Then there's another problem, when I go CMD+ALT, the switch screen won't pop up, and the Force Quit menu is empty too.

Add to that even when I have the "Show all file extensions" unchecked I still see all of them, I've tried checking it and then unchecking it. no luck.

Any help here would be nice.

13" MacBook (late-2008)
Reformat the drive (hfs+ extended journaled) and reinstall, not just the system files. and don't import any old user accounts.

Isn't there a simpler way to do it ? I mean so that I dont have to reinstall it.. I realized the apps was my problem, I had his backup copied on. so it was fetching from there. plus I did reindexing of spotlight, but the other problems still stay..
