Quad G5 open firmware hanging

Does anyone know what could cause open firmware to hang?

In one example below "reset-nvram" returned ok but open-firmware hung as I was entering "set-defaults".
"reset-all" sometimes hangs at the erasing line, othertimes at the flashing line.

I can just be typing commands like "boot cd ..." and it will lock up.

I've also seen it hang when entering "setenv auto-boot? false". If the command does work, it doesn't see to be making it to flash as on a subsequent boot and manually forcing a boot into openfirmware if I run "printenv" auto-boot is still true.

I've replaced the 3v 3022 battery. I've tried option-cmd-p-r. While I'm holding it down I can hear the machine reset, but it's 50/50 if I get the chime following.

I've tried pressing the SMU Reset button (next to the battery). I've tried holding power down thru the programmer tone and chime forcing into openfirmware.

I suspect a hardware issue :(
