While viewing a youtube video my Imac shut down and off. Had to unplug the machine and wait a while before it would restart.
No error messages after the boot. Ran the first aid on the disk:
Verifying file system.
Volume could not be unmounted.
Using live mode.
Performing fsck_apfs -n -l -x /dev/rdisk1s1
Checking volume.
Checking the container superblock.
Checking the EFI jumpstart record.
Checking the space manager.
Checking the object map.
Checking the APFS volume superblock.
Checking the object map.
Checking the fsroot tree.

fsroot tree is invalid.
The volume /dev/rdisk1s1 could not be verified completely.
File system check exit code is 0.

Operation successful.

Hard drive is at 299 gb out of 499 gb.
The data about the machine is correct on the side bar.

From the report it seems like the issue was repaired successfully. Try running Disk Utility again. If it cannot unmount the Disk you could try running it from Recovery mode.

The side bar on the left has all the info and it's up to date.

Okay, I did a bit of a search on "
fsroot tree is invalid.
The volume /dev/rdisk1s1 could not be verified completely."
I found a lot of articles all dating back to macOS Sierra, High Sierra and Mojave.
This was around the time when Apple swapped from HFS+ to APFS and that may be meaningful.
See; fsroot tree is invalid after Mojave insta… - Apple Community

The attached article describes a number of methods to fix this but sadly doesn't list the repercussions of doing nothing although some reference to Time Machine was made.

Worth case scenario sounds like cloning the data, erasing the HD and cloning the data back. This is pretty drastic but if it comes down to a formatting issue it may be the only way.

In any case it warrents a bit more research. Sorry I have no real solution at this time.
Here is another one; Disk Utility error (fsroot tree is invalid.)

Started up in recovery mode, disk utility could not un-mount the volume

Thanks Rod, I'll go through your links.

No problem, I'm sitting in bed (hence the iPhone) recovering from a double hernia repair. So, I'll do a bit more reading on the topic.👍🏼

I would suggest a full backup asap either with time machine or a clone eg Carbon Copy Cloner (my choice).

Yes a full back up is needed. I've got a copy of CCC and a spare portable 1T disk.

Sounds perfect, I'd suggest you use CCC'S option to create a bootable clone if it's not already. If your only alternative is to erase an reformat your internal HD a bootable source will be a necessity.
Should you decide to go that way please feel free to ask any questions that may arise.

It's not unusual to have issues as you have found on an older iMac. This issue may or may not have anything to do with the shut down you reported. At the worst you could have an intermittently failing power supply, fairly common on the older 21.5" iMac's. Also a sudden shutdown can cause system issues, the results of this could be what disk first aid has detectected. If there is data you value on your iMac a backup or clone is a must.

The CCC is backing up.
If reloading the backup files doesn't fix the problems I'll install the OS, have bootable DVD with High Serria 10.13. If that fails to clear the problelm I'll run the Apple hardware diagnostic.

I did not see a switch for a bootable clone, it might be automatically bootable? I also have backups on “time machine” so either way I'm covered.

The bootable dvd I bought is a scam, will not load.

But did find this interesting, on my Disk Utility I have at the top level "samsung ssd 840 evo 500gb media" Ran the Disk Utility and it passed!

Mid level is the "container disk 1" When and where this came from I don't know. It failed on the Disk Utility with the same errors as the "machintosh SSD"

The bottom level is the "machintosh SSD" which failed the disk utility as before.

Before I go any further is that mid level "container disk 1" a problem?

No. It's part of the APFS file structure.

The earlier versions of CCC create Bootable clones by default. It usually says something at the top like "all things being normal the clone should be bootable."
You can test this by starting up the computer from off with your EHD connected and holding the Option key down. The result should display boot options, one should be your clone. Choose that and ensure it starts up. It might be a little slow. If successful just choose Restart from the Apple Menu and it will return to boot from the internal drive.

It's bootable!

For now I'll keep my backups going. I get my internet connection is Wi-Fi, trying to download software is hit and miss and the closest apple store is 80 miles (128 km) away.
Thanks for the help
