Hi to make sure, what do you do to delete the file? To move to trash, you press Cmd + backspace.
To ensure however the file is permanently deleted i.e. not moved to trash, you press Cmd+backspace+Alt
Is it possible otherwise your alt key is stuck down if you are not pressing it? To test this, try typing the number 3 and see if a # is displayed instead.
