I've got an iPhone on 10.26.17 and received this email from Apple support:

Checking the headers of this email show that it did come from Apple:

Except I did not call Apple support, don't even know the number for them. My Apple ID 2FA had been enabled on, with the help from Lisa (Mac-Forum), on 10.27.17 and confirmation sent to my email address:

The only reasonable explanation seems to be, that the carrier store sales guy called Apple for whatever reason. Is this correct, or there's something else going on?

PS: In either case, I changed my Apple ID password....

Either which way, it's just a survey that you can safely ignore and having changed your Apple ID password you should be fine.
