Mojave 10.14 / iOS 12.1 - Is it necessary to use Apple Keychain in iCloud? If using 2FA must Keychain be used?

None of these are musts, but you can use Apple Keychain in iCloud if you want to sync across multiple devices. 2FA doesn't rely on Keychain at all, it's totally separate.

When you say, "can use Keychain if you want to sync across multiple devices". I only sync Notes across multiple devices therefore is there any other reason to use Keychain? (My email is IMAP so each Mail App on each device syncs with the IMAP account so I don't need to sync it with iCloud.)

Keychain holds your passwords and other credentials. iCloud Keychain is the process whereby you can now have access to those things on multiple devices. Other apps like Notes and so on ALSO use iCloud to sync across multiple devices but do so independently.

Is it necessary? No. Is it very handy? Decidedly yes! But it's your call.
