I have also noticed previously, that sometimes multiple SMC resets were required to fix some issues. At least, on my 15" Late 2011 MBP they were needed.

So I ran Onyx like you recommended and waited 2 weeks to see if anything changed, and wadyaknow! No fan issues since, so I'm going to assume the issue is fixed, thanks!
