I have had exactly the same behaviour since installing Catalina. I now automatically double click the icon when first launching the program!
Yes, I am in iCloud, iCloud is set for Books and Books on the iPad is set as well. Here are my settings Mac: System Settings, AppleID, iCloud
Did you log on to iCloud and sync with wi-fi on?
I don't know. I have Catalina 10.15.2 and iOS 13.3 on my iPad and they sync, as I said. I don't know if Mojave can sync with the latest iPadOS version.
