How do I get rid of the ? on the finder bar?

MacBookPro 15", Original iPad, iPad 6, iPhone 5
Fairly new to Mac and I have most things figured out but this. On every finder screen in the top grey bar there is a large question mark. When I click on it, a file name appears in the bar (it's a .pdf) but then I get a message which says "Item cannot be found". I don't want this file (don't even remember it) and would like to get rid of the question mark. Any suggestions? Thanks.

Hold down the Command key, click and hold the mouse button down on the ? and drag it to the Desktop area, release the mouse button first, then the key. Poof!
