Need help with a Marketing Template

Hey guys,
I have done a bit of google searching and will do some more, but I wanted to see if anyone in advertising might be on the forum that could help. I am working for a marketing dept. on the campus where I am a GA. We have several events each month and we are responsible for getting the events publicized in local newspapers, radio, tv, online calendars, etc.
i am wondering if anyone is in a company that does the same that has access to a template of sorts. We want to be able to hand our clients a piece of paper or direct them to a website where they can put checks in the boxes for what services (radio, tv, etc) they want us to do for their event. Then we will take those requests and form an estimate for them.
Does anyone have a model of this that we could use for reference?
