I have been searching for hours trying to find someone with your exact issue. I myself have never seen it on the many Macs with OSX I have here and from 10.2 - 10.5.1. The only thing that I have found is about issues with the Keychain causing your issue. Try opening KeyChain Access from Applications/Utilities and click on Keychain First Aid from the Keychain access pull down. Also look and see if any of your sites info is saved in the Keychain. Hope this helps.
