Extract subtitles from dvd

I need a tool to extract subtitles from a dvd and save it as SRT, SUB or something similar.

I tried D-Subtitler but it just stays "converting pictures to text...". It either doesn't work or takes WAY too long, the ripping (using mediafork) has already finished and it still hasn't extracted the subtitles.
