iTunes 12 not syncing properly with my iOS devices

2009 MacBook Pro 15" - 2013 iMac 3.4GHz quad-core i7, Fusion Drive, NVIDIA GeForce GTX 680MX
I have IOS 8.1 on both my iPad Mini and my iPhone 5 and iTunes 12 on my iMac. I change what songs or videos I want synced and iTunes seems to complete the action but when I look on my device that don't match. TV shows are on my iPad that I "unsynced" as well as playlists on my phone? Worked fine before the most recent upgrades of iTunes and iOS; not sure which upgrade broke the syncing (or if it was the combination of the two).

Anyone else having this issue?

iTunes 12

When I sync my iPhone 4, iTunes kept adding unwanted apps. There doesn't appear to be a way to delete old apps from iTunes 12.

I'm now worried about trying to sync my 5S.
