Please suggest an alarm clock that will:
take at least fifteen alarm settings
will control the sound to stop after five seconds​

I want the alarm to simply notify me, not continue until I have to turn it off.

That's more a repeating reminder rather than an alarm I'd suggest

Assuming you're using iOS6, create a 5 second 'alarm' tone of your choosing, use that for reminders and use the reminders app to create a repeating reminder
