My iPhoto library has about 20 000 pictures and I have been reading about creating multiple libraries. iPhoto is slow to start and takes its time when using it.

Running 10.8.3 with 8GB of RAM with 35GB free out of 500GB. iPhoto version is 9.4.3.

I have not tried some of the recommended things from other threads, such as this list from here.
1. As always, make sure your backup is up to date.
2. Launch iPhoto with the option and command keys held down.
3. Check every single checkbox. Leave it alone for a while and let it do its thing.
4. Not required, but I like to restart my machine after this.
5. Fire up iPhoto normally and see how it does then.

But...if I do create multiple libraries how does it work when I want to attach a photo to an email? I know that I can send photos from iPhoto itself but my work process is to attach them from Mail. Checked the Apple Support pages but nothing answered my question.

Give iPhoto library manger a go, it might be a better solution than constant restarts

The other reason it could be taking a while is you only have 7.15% of free disk space on your 500GB HD . . .
The rule of thumb around is to have at least 20% free space depending on what your doing. For such reason for the system to work at its peak, swap file, and so on. I suggest Backing up and deleting some stuff and clear up your HD. I reckon 20 000 images would be taking up a lot of that space, but it would be advisable because sooner or later the SBOD will arise and you will need to force shut down . . .

Just a suggestion

It works fine. When I need to do this I just "grab" the photo(s) in question from iPhoto and drag them onto the Mail icon in the dock.

I know that I can send photos from iPhoto itself but my work process is to attach them from Mail. Checked the Apple Support pages but nothing answered my question.

This will also work fine. Mail uses whatever iPhoto library is set as the default. When you change libraries, that becomes the new default until you change it again.
