imac late 2009 crashes on wake from sleep

Have had problems last 2 weeks iMac crashes from sleep seems most likely scenario, taken off password, reinstalled OS (Mountain lion) and disconnected all peripherals in turn.

On verify disk get quite a few errors, even after repairing permissions (invalid directory item count, volume corrupt - repair disk greyed out. I am regularly backing up!

Any ideas? Hard disk failure coming up? or software.
