Do you have VLC installed? If not, download and install it from here. It's free and should be able to open and play your MP4 video.

Main-11" Air, iPhone 6+, iPad Mini 3, Hi-Fi Extras- Too many to count
Try using VLC or uninstall and then reinstall QT and see if that helps.
