MacBook Pro freezing up

This is about an older MacBook Pro which was running beautifully until recently when it started freezing up, badly. The sad thing is this was my only Mack machine that I could depend on when my other two Macs, including my Mac Pro tower with Cinema display would often and still do freeze up. More sad is the fact that Apple is loosing its legitimacy as a superior product.

Although I have not installed anything new or changed anything recently, my MacBook Pro will now freeze-up at various times, and I can only shut it down by the power switch.

For the last five years I have installed all the major graphic & video applications, 140 applications in my 'Applications' folder without any problems, of course running only one application at the time.

When it freezes up only the cursor arrow is movable, but nothing is selectable. At other times the whole screen is covered with black & white broken-up horizontal lines and within a few seconds it goes black while the fans are running wildly, and the computer shuts down on its own. Some times, but not always, the fans start to run very high and noisy just seconds before the freeze-up.
And yet, I've been working for ten or more hours at the time in 'TextEdit' and 'Pages' without any problems.

For more than two years I have been using my MacBook Pro at home 99% of the time, connected to a 42" Samsung monitor and it is always plugged into the wall with the battery indicating a 98% charge.

Typical freeze-up instances are;

1- While in 'Photoshop, CS4', at any time.

2- While in 'Preview', viewing .jpg images.

3- While in 'Adobe Acrobat Pro' combining .jpg's into .pdf.

4- While in 'Safari' Version 5.1.7 (6534.57.2). Same with 'Firefox' v.18.0.1 and 'Chrome'
15.0.874.106 .

5- While playing any video file in VLC, QuickTime Player 7, or any video player it
freezes within the first minute of play.

I suspect two causes that might tie-in with each other. One is the video card being either weak/faulty or malfunctioning at higher temperatures. The other is the possibility that the fans are either malfunctioning due to electro/mechanical reasons or that they, and the whole interior of my MacBook Pro, are very dusty preventing the fans from circulating the air efficiently, thereby causing overheating when the CPU is called in for heavy action.

I have installed 'smcFanControl' set at 4,075 RPM for both left & right fans, and it indicates an average operating temperature of 140 degrees F at 4,000 RPM, although when running applications it can climb up to 216 F & 6,000 RPM.
Most of the time the fans run quietly even at high speeds, but at other times they run very noisy, almost always followed by a freeze-up.
My system freezes-up even when the temperature indicated in 'smcFanControl' is around 195 F or lower, could it be that this temperature is too high for the video card ?

I have also done a clean install (after zeroing out the whole drive, EFI & GUI) to no avail. I have booted from an external drive as well, with the same problem.

Following the instructions from one of the Apple community threads I have also issued in 'Terminal' some analytical commands, included here below with their results, which I do not understand. Could someone please decipher these for me ?

Firstly, what version of OS 10 are you using

And make sure you have run Disk Utility and done a permissions repair and verify the disk while you are there

Also, how much spare hard disk space do you have?

I've done everything you mention here, and I am on Mac OS X 1.6.8 SL with a 75% free HD.
I am trying to find out if the video card could malfunction by an overheated interior during high processing demands from apps like Photoshop etc.
