Could it be that my MacBook 6.1 is dying on me?

Hey guys! I have a late 2009 White Unibody MacBook 2.26GHz with 4GB of RAM and an nVidia 9400m GPU. I've noticed over the past several months that the laptop get VERY slow. I have reinstalled Snow Leopard SEVERAL times. I even tried Lion and Mountain Lion to no avail. This is not an issue with other applications because this happens right after I finish installing the OS. I don't do anything intensive on the MacBook except for a little bit of Photoshop.

Is it possible that the MacBook is dying out?
