Insert the disk, reboot and hold down the C key as soon as you hear the boot chime. It should boot to the disk where you can now reinstall OS X. Be advised though that if the kernel panics are being caused by defective hardware, reinstalling is not going to help.

The drive is not mounted because you booted your MBP from the disk. Highlight the drive on the left side and then select the "First Aid" tab on the right side. Do a verify and repair on the hard drive. If it shows errors and repairs them, do it again until all errors are corrected. After it's done, select repair permissions and allow it to do that. Report back the results.

This is what i got when i did verify:

Verifying volume "Macintosh HD"
Invalid content in Journal"
Checking Journaled HFS Plus volume.
Checking Extents Overflow file.
Checking Catalog file.
Checking multi-linked files.
Checking Catalog hierarchy.
Checking Extended Attributes file.
Checking volume bitmap.
Checking volume information

The volume Macintosh HD needs to be repaired.
Error: Filesystem verify or repair failed.

then i did the repair and this is what it looks like:

Verify and Repair volume "Macintosh HD"
Invalid content in Journal
Checking Journaled HFS Plus volume.
Checking Extents Overflow file.
Checking Catalog file.
Checking multi-linked files.
Checking Catalog hierarchy.
Checking Extended Attributes file.
Checking volume bitmap.
Checking volume information.
Repairing volume.
Rechecking volume.
Checking Journaled HFS Plus volume.
Checking Extents Overflow file.
Checking Catalog file.
Checking multi-linked files.
Checking Catalog hierarchy.
Checking Extended Attributes file.
Checking volume bitmap.
Checking volume information.

The volume Macintosh HD was repaired successfully.

The problem is I've done the verify and repair five times and it says the same thing everytime.

This is what it says:

First Aid Failed

Disk Utility stopped verifying "Macintosh HD"
because the following error was encountered:

Filesystem verify or repair failed.

Reboot and hold down your mouse button. The disk should eject. Also, it very definitely looks like you need a new hard drive.
