Cellphone Carriers Charging for iMessage?

To all,

Has anyone experience their cellphone carriers such as AT&T, Verizon, and Sprint charging messages sent via iMessage as SMS even though

1) The text bubble is colored blue instead of green
2) Send as SMS feature is set to "Off"
