Mac Pro issues

I own a 8-core 2009 Mac Pro 2.26GHz 8 Core processor.

The other day I booted it up and after logging in the screen would go black and the internal fan would become very loud. I have tried the following:

reset pram & smc, reseated ram, swapped the internal hdd drive with another system drive that only has Mac OSX 10.7.3 installed. Unfortunately none of these fixed the problem. I managed to login in long enough to pull off a crash error report and this is what I got (this is from the original system drive that was running 10.6.8 not the 10.7.3 drive).

If anyone has any ideas that would be great but im guessing its hardware fault - logic board maybe? There is hardly any dust inside the computer so I have ruled that out being the culprit.

Possibly logic board or hard drive (corrupted driver, etc) OR - this is where I'd start troubleshooting - possibly the NVidia graphics card.
Try re-seating the graphics card (just a little jiggle in place), and if that doesn't help, try another graphics card.

as a suggestion open the machine up and give it a good clean out. Mac Pros with the five fans do accumulate lots of dust and fluff. Then remove the gfx card, clean it well, and re-install. Suggest first trouble spot from your description is the card. GeForce is mentioned earlier in the log report so well maybe a sick nVidia card.
