Creating Forms in Powerpoint?

I'm creating a Powerpoint that would be sent out to multiple clients, and instead of having a "Call me today!" slide at the end, I wanted to put a form in there that would just ask for their first name, last name and email address if they were interested, which would then be ultimately emailed to me... Is this even possible? Anyone have any ideas of how to do so?

A form requires more than just input fields. It requires a submit button that starts a script that handles the data and sends out the results through a mail server.

You maybe able to embed a word form in PowerPoint, I dont know, but then you still require the mail out function, and even if the word embed works the user would have to save the file and email it back to you manually.

Web forms require that the web server runs a script that handles the form data and interfaces with the webservers mail server to send the results of the form, so all that tech is based on a server.

You would be better off with a straight hyperlink that links to a web form on your company's site, where the form can be properly handled. Expecting the user to fill out a form locally on their machine, save a file and then attach the result to an email may just be too many steps and put most users off bothering
