In pages '08 I could open a new document in full screen mode. But '09 doesn't seem to offer that option, even when I set the default to 200%. Any clues?

What version of OS X? Pages '09 supports the new full-screen mode for Lion. If you aren't running Lion, then it's likely that the old-style full screen mode has been dropped.

I am running Lion. I am referring to initially opening pages app.

You mean the Template Chooser? No, it doesn't appear to be possible to start it in full screen mode. Why not just click the green button up top to maximize it? It will remember to stay maximized next time you run Pages. You can go true full-screen after selecting a template if you like.
