Today I took my Otterbox off of my iPhone 4 and I noticed that the water sensor in the headphone jack was tripped red, but it is only the size of the tip of a pen. I have never had my phone near water and I do not know how this happend.

It's likely moisture (probably humidity) condensed inside the Otterbox thereby tripping the moisture indicator. As far as voiding your warranty is concerned, it depends.... By that I mean it depends on when and if you take your phone in for repair, the person who is doing the repairs will probably be the one to make that determination.
