How do I make the speech start somewhere in the middle of the pdf, instead of starting all the time in the beginning.

Oh cmon, it can't be that hard. Just tell me please? Or point me to a web page that would teach me. I tried to google it, but couldn't find anything.

What happens if u highlight the area that you want and then start text to speech? or place cursor at start point.
