MBP Unibody "Black Screen" Problem

So recently, whenever I leave my MBP open and go away from it (instead of closing it) I sometimes come back to a black screen. The power is on, but clicking any keys, the trackpad, or my USB Logitech G5 mouse doesn't rid it of the "sleep-like" mode. I don't really know if it is sleeping or not, that's just the only thing I can describe it as.

I try closing the MBP and leaving it for a while and then coming back to see if it's fine, but that doesn't work either. I find myself hard resetting (holding the power button) every time this happens and I feel like that can't be good for my MBP. Upon restarting the computer I get a "Your computer has shut down unexpectedly" report and when I send the report I get these details (sorry for the length):

Looks like Kernel panic, you might try running disk utilities from your restore disk. You should put the disk in before you turn off the computer, then restart. It should come up to the first install screen, at that point look up at the menu bar and click utilities. Run disk repair and then repair disk permissions. See if that fixes the problem. I use a program called ONYX for Snow Leopard, it runs all the built in maintenance programs to keep everything smooth.

Thanks cameraman, but that didn't work.

I read the panic report more carefully and it says things about my Graphics Card under backtraces. Could it be that it is damaged?
