Check your LCD to see if there is an option for "Auto Correct" or something like that.

System Preferences - Displays

You'll have 2 windows, one for your MBP and one for the Dell. Set the correct resolution for the Dell.

Here's the deal. I use an external keyboard and mouse. Plug in the 22" and start the MBP. When it boots up, close the lid. Let it sleep for a moment. Then wake it up with the keyboard/mouse. Keep the lid closed. When it wakes up it will automatically adjust for the resolution needed for the external monitor.

If the monitor is working, then there will be a 2nd window in displays preferences.

If you are in extended mode rather than mirrored, the window for the Dell will be displayed on the Dell.

If you have 'Mirror displays' checked, then try moving the window you're looking at to look behind it.

edit: There has also been a firmware update for the mini-display port to VGA adapter. To install it, you need to have the adapter connected with the monitor turned on and run software update from the Apple menu.
