Just updated to 10.3.9 and am wondering if it's advised to repair permissions. TIA

You really should repair permissions after every software update. Also about every two weeks or so, even if there wasn't an update. Just one more way to maintain your machine!

Should I use disc utilities from the startup disc or just in my utilities? Whats the difference between the two?

As a mac user, you'll definitely want to get in the habit of verifying and repairing permissions on a regular basis.

You can just use Finder>Applications>Utillities>Disk Utility

It is the same app as the one on the CD, but you won't have to use the CD to run it.
