Highlighting in image-pdf's

Hi there

I'm looking for a software with a functionality to highlight passages in an "image" pdf-document (i.e. one that is not recognized as text). It wouldn't necessarily have to be a proper pdf-reader, it might also work with a simple drawing app capable of opening pdf's and a transparent paint brush tool.

Any ideas?
