Duplicate restart, shutdown menus, strange flashing icon

I just turned on my iMac and noticed that an icon on the desktop has a flashing black box around it and in the mac menu there are two restart, shutdown and log off menu items. If a dialog box pops up the OK button flashes every second accompanied by a clicking sound. It's as though a key is stuck on the keyboard but I have unplugged the keyboard and also the mouse and every other peripheral attached and it is still happening.

Has anyone seen this before?

uncheck Voice Over

command + F + 5

turns this on and off also
