When I go to restart or shut down, there are two options

I am not sure what is wrong with my comp. When I go to restart or shut down, there are two options (duplicates)!! Also, in each program, there is a box around some random thing (See pic). What is going on??? I haven't done anything since this morning (it was fine)! Any ideas?? i've restarted and shut down, etc, and it is still there.

You've turned on Voice Over

It's located in System Preferences - Universal Access

Also has a shortcut - pressing Command + F5 - will turn it on and off

You probably just hit command-F5. I've done it before. If that happens again, just push command-F5 again and you should hear a voice telling you that voiceover has been turned off.
