Password authorization window disappears

Greetings, all.

A couple of times recently the password authorization window has disappeared suddenly just as I was about to type in the password.

This has been happening in the middle of software installation, preventing me from completing the installation.

Any idea how to stop this?

Lately while I've been attempting to install software the Password Authorization has been abruptly disappearing before I can type in the password.

Any idea how to fix this? It's preventing the installation of updates.

I'm running OSX.4.11
