What the heck is going on?

Well something is going on with my MacBook, I came home today from work, and my computer is acting up. Well as you can see from the screen shot, there is double logouts, double shutdowns, and double restart options. Also, whenever I click on a folder on my desktop, there is some double square that comes around the folder I'm picking, as you can tell from the screen shots.

I've tried restarting, and i've tried shutting down, so if someone could give me some options, please help me!!!!!!!!!!!!!

1. The below solved it on mine.

get Onyx and use the 'Rebuild LaunchServices' under the automation tab

If you have Main Menu, pretty sure it has this option also.

2. For the box - Go to System Preferences, Universal Access and turn off Voice Over

Is that really voice over? That is the only thing I didn't think of, thanks man. I'll let you know how it turns out!
