OK so on my website so that you can have people write stuff on on it and leave a comment. Now I used the following code once to test out a phishing website (only out of interest to see if it works):

And I want to use this as the action of a form that relates it to a file in my domain that displays what has been written on my web page. Can anyone tell me the parts I need to alter?
