Need help w/ Javascript on my blog

For the last few weeks I had a great script working that embedded a search box so users could search all my posts. However, it's suddenly disappeared. Here's the javascript I used:

Ziked - Installing The Perfect Tumblr Search

And the search box was right above the RSS FEED/ARCHIVES links in the right column. Any ideas on what happened to it? I haven't messed with the code since.

Hi I'll post on this again because I'm on my iPod and I can't view the source code but you could try and work your way through till you find a mistake maybe it doesn't support the browser I really don't know because I don't have the source code
