Membership number?

My iMac's HD is dying (after only 6 months) and I need to make a reservation to take it in to an Apple Store. In order to do so online, I'm asked for a membership number. I have NO idea what that is or where to find it. Any help? Thanks!

Make an appointment as a guest, not as a member.
