OS 10.4 - Macbook formatting problem...

I formatted my Macbook today, and it worked well... I thought.

Now any new information or programs I install are lost and also not available. By this I mean that VLC (for example) is not in the Documents folder, but if i spotlight it - it's there.

After restarting my computer, the dock had resized brought back all the icons I removed and gone back to factory settings (the trackpad wouldn't allow clicking again)

It seems like i'm creating different user accounts, because when i try to change the name of the computer it will allow me, then when i restart, the name has reverted back - and I can't use that particular name anymore as it says it's currently in use.

Any ideas?

Try another format? Very weird.
