    How do I accomplish this on Mac OSX?
    An app like "Time Counter" for iOS?

    This app to be specific:

    Or something that sits on the desktop like a widget? Or something in geek tool?

    Firstly...did you do any sort of internet searching before posting a question? I found this app in about 5 seconds:
