Hello

I need to fill this PDF form , someway i don't seem to understand how to fill it,i mean they expect me to print the form after i filed it by editing the file with the computer(except the signature part of course) and the little blinking line is visible and moveable but you can't write..

I downloaded the last version of Adobe Reader , didn't help

What to do ?

Did you see the instructions on the form itself?

Please make a copy of this entire form, including the copies of your credit card above and a government issued photo ID on the following page, and fax to 815-337-8208.

Meaning, you print the form out, fill it out (by hand) and photo copy the filled out form with copies of the credit card and ID where indicated. It's not meant to be filled out electronically.
