Try this: create a new User Account from the Accounts System Prefs. You can delete it later. As a test to see if there is some conflict within your main user account, create the new one, log in to it, launch Safari and navigate to a web site with video to see if the audio plays. If so, then your problem is in your main user account. We can talk about what to do after you reply with the results of your test.

Played any games lately? Try this:
Go to /Applications/Utilities/ and run Audio MIDI setup.
Make sure input and output are set to 44100.0 Hz.
Sometimes games or other apps change this and you wont get any audio.Always listen to experts. They’ll tell you what can’t be done and why. Then do it!
