MacBook - Won't startup

hi there,

i have a macbook, it's been a great machine. Never given me any problems until today. I tried to strat it this morning without any luck. I heard the start up tone, then next was the grey screen with the apple logo and the loading icon, this would last for about 2 min then the systme shutts down to a black screen, completely dead.
I have tried a safe boot, will not work
I have tried holding down the D key on startup, no luck either. I have insterted the osx install dvd and tried to load from the disc and that didn't work either.
I also ran the disk utility to verifiy the disk permissions and also verify the disk and I get the same error message.
Error: the underlying task reported faliure on exit.
