MacBook - My mac is messed up?

Ok...so I'm not too bad with mac's. But I'm not the best.
I was on my mac earlier, and suddenly I started getting this random black box around everything. Also my apple start button thing, has double shut down's and restart's....
Anyone know wtf this is all about?

Also, in my activiy monitor, there's an activity called "screenreaderd" that keeps saying it's unresponsive...And it only does it on my account.

I seem to have found out what it was.

I google screenreaderd and found out it was something the the speech settings in the system prefs. I changed it, and all of the box things went away.

Anyone know why it was doing it? O_o

You probably hit command-F5 which turns voiceover on/off.
