Power Mac - Duplicates menu options???

Hi i am currently on a work placement as an helpdesk admin and have come across a problem with an individuals g5 mac. There seems to be duplicates on the apple menu e.g. two shut down options etc and also when an icon is clicked it has a black box around it? I have resetted the mac and also restarted it in safe boot but same problem.
Could someone point me in the right deirection please.

Found the solution. VOICE OVER WAS ON. JUST NEEDED TO BE SWITCHED OFF LOL
