My DSL modem keeps dying, help?

For some reason my DSL modem keeps dying on me, I'll be downloading for awhile or something and then my internet stops responding and I have to restart the modem, then it works again. Sometimes I won't have to do this for weeks, then somedays I will be restarting it every other hour. This morning I looked at the modem after it died again, and only one light was on, (usually 3, blinking). Do you guys have any ideas on what is going on? Is it my ISP, or my modem, or what? It's connected to a D-Link DI-614+ wireless router/firewall. (excellent router!)
