Digital A/V Adapter Issue

I bought a Digital A/V Adapter for my iPad 2 a few months ago, and when watching tv shows/movies on the videos app, the video cuts out. The screen will momentarily stay black, come back on, but no audio.

At first I thought, okay, this is probably just the TV. Its an older Hitachi plasma. I hooked it up to my brand new Samsung LED TV, and the same problem exists.

Any ideas? This problem is very frustrating and ruins the suspension of disbelief when watching movies!
