I used to use Appleworks, a wordprocessing document custom-made and carefully formatted and mail-merged using an Appleworks DB file for information that plugged into the slots. Actually, I had to break my employee list into two, one for the upper half of the pre-printed form and one for the lower half.

What do other Mac users use for this job. Appleworks no longer works for me for this job... now the fields vanish from the document just as I hit the "print" button. They reappear after I close the document and then re-open it but this can get expensive when two tries runs through one pack of the pre-printed forms.
